I’m new to zbrush, and can’t seem to get the symmetry working the way it should. I’m not sure how to set the pivot precisely other than just going into transform and tapping the spivot twice. I am using zbrush 3.5 r3.

One option is to hide all but the part of the mesh that you want your symmetry to work around and then activate Transform>S.Pivot. This will set the pivot point to the center of the visible mesh. You can then unhide and all symmetry and deformation for the model will function relative to that pivot point until you press C.Pivot.
