This is a concept of mind which one brings a species, known as “Watcher Skull”. Members of this species can see everything what all members are seeing. Something like a entire web of watchers.
This model represents a ritual made by a cultist. In this ritual the cultist trade one of his eyes for one eye of the “Watcher Skull”. With this ritual the cultist can see through all the eyes of every “Watcher Skull” creature.
