Surface Noise: apply to mesh = weird deformation

Having an issue with surface noise. When I hit apply to mesh on the surface palette the entire mesh deflates a bit. Not sure what the issue is, seems like it might be some kind of offset or surface normal problem; but I’m not sure where to go from here.

I got something similar - the noise applied to the mesh kinda smoothed out and is bulby
