Subtools wonkiness

In the subtool thumbnail (and subsequent flyout image) it appears the mesh has overlapping geometry… that doesn’t show in the actual model in the workspace. Does anyone know what this is and if it’s related to something I may have incorrectly done?

In a possible related problem is that on my last increase in mesh density that I lost my lowest level. Did I exceed a threshold of some sort?

Thanks in advance,

look like you have there two identical subtools…duplicate unnoticed?
masked?

Possibly, but how do I figure it out? I noticed that when I appended eyes to the subtool palette that the visibility buttons were acting strangely (e.g. If I turned off the eyes, everything else disappeared). This behavior was inconsistent to boot. Is there another way to see all objects/tools in the scene like Maya’s outliner?

I find this often happens with picture representing the mesh in tool box. The model appears to be only partially covered by the material. I just ignore it as actual model is unaffected.
