#1) Update your software. #2) Don’t crop off critical parts of the UI when you want us to guess what may be wrong.

What you are doing is sometimes tricky at first but you will get the hang of it.
It looks like the top part needs more lines, basicly you have not yet described enough edges for it to draw full polygons.
A polygon generally has three or more sides, usually three or four.
When doing retopo with ZSpheres (and some other methods) you have to make sure that each polygon you are creating is a closed loop.
Using the letter A as the video suggested is good, do it often, like right after you make a polygon to see if it worked. If it didn’t UnDo and try again.
Using Move can complicate things, try to use it when you are looking straight at the surface you are moving a vertice across, it can put them in odd places otherwise.
One last thing:
The pictures you provided, by the way, are fine, you didn’t do anything wrong.
Also, not that you need permission, you can update your software when or if you see fit.
Alec, your screenshots are fine, don’t worry about them at all.
I’m still wondering what critical parts were cropped off.
