When Use the symmetry in the X Axis … it Becomes Really Crazy !

Pressing Shift while you rotate the view will snap you to an orthographic view, ie front, back, side, top, bottom. You are just not snapped to the front view.

and the symmetry will work just fine ?
There is nothing wrong with the symmetry, it’s just the way the tool is positioned on the canvas. Turning on the “PolyF” button on the right shelf (or Shift+F) will help you to see how the tool is positioned. The XYZ Tool Position Axis plugin will take the guess work out of it. Note: The plugin post say’s ZBrush 3, but works just fine in 4R6.
