Under Preferences > Tablet, the buttons are grayed out and the my pen seems to have no pressure sensitivity?
Using an intuos4, zb4r6
Any help appreciated.
Make sure to install the most current drivers from Wacom. The default drivers that are part of your operating system don’t have everything ZBrush needs.

Thank you, I submitted a support ticket to pixologic and a chap called Will said the same thing (Very prompt response too). I had the latest drivers for my wacom, however where I went wrong was that the procedure to obtain the latest wacom drivers is to first completely uninstall the old driver, otherwise a conflict arises. That is what happened to me.

For me it was a case of removing a file called wacom_tablet.dat (On a Win7 64 Bit system)
Reinstalling the wacom driver and rebooting, then the zbrush Tablet panel is active, Yay!
So, in short it’s a wacom issue, but I wouldn’t have figured it out without the support of pixologic. Thank you Will and aurick, hopefully this thread will help anyone else with the same problem.
