This is a pic of the male mesh( divided ) that comes with zbrush, but the problem happen on my meshes as well. My meshes at some point always seem to get these little pin point extrusions( location is random, on my mesh they are on the head ). They can’t be smoothed and regardless of the pic they do stand out. I though it may be because part of the model was masked, but it wasn’t. Any ideas how to prevent or fix?

If i’m not wrong this is because one of the vertices have only 3 instead of four connections. This can also happen if it has more than 4. These are named “poles”, and the best way to “get rid” of them is to place them where you wont see them. Because you cant really make a mesh without them.
So when you’re retopologizing keep this in mind, try to make as few as possible and place them on places that’s not visible in the final render.

One thing you could try is using the smooth valance brush. It has been developed for the purpose of dealing with poles.
And yeah you will need to get into retopo
