Opened it in Maya and added couple of loop edges around the lips. Saved it, imported back to Zbrush with morph target set and the model explodes.

I’ve had that happen to me before too.
What I do is hide most of the good mesh, and then use MOVE and SMOOTH to repair the damage.
Ok from my experience Im pretty sure this is whats happening…
See when you export a mesh, edit it, and re-import it back into the lowest subdivision level what you are effectively doing is running a project all (the option in the subtool menu).
Im assuming it asks you if you would like to reproject the details when you import it back in?
the Dist slider next to the project all button determines the distance zbrush looks for a surface to project onto, try turning it down to a low setting and importing the mesh again.
I would also advise importing the fixed model as a subtool and using the project all manually, that way you can tweak it easier by simply undoing, and projecting again with different settings.
To sum it up, lower the Dist setting of the project all menu under subtools.
I have some other advise on the subject, but I want to make sure I nailed what the problem is first.
Thank you all for your feedbacks. Here’s what I just found out and tested:
I exported the mesh into Maya to add the loop slice and imported back to Zbrush -> Explosion
I exported the mesh into Silo to add the loop slice and imported back to Zbrush -> Explosion
I then imported back to Hexagon, in which, was the app I used to model the mesh and imported back to Zbrush -> NO PROBLEMS!

I don’t really like to use the project all, as I think is troublesome. Though it could be a solution never the less. But after testing it as described above, I find it weird and if Maya or Silo is not messing up the scale… At least I set it to 1 before exporting it, under Export tab in Tools.
