How do you mask along a spline in Zbrush 4 ?

You can use the options in the Stroke palette. Make sure that you hold Ctrl while selecting them. The attached brush preset shows how. Unzip and put it in your ZStartup/BrushPresets folder and it will be available as a new Masking brush - called MaskPenSpline - whenever you start ZBrush.

It is a modified version of the MaskPen brush with these Stroke palette settings:

  • LazyMouse on
  • Backtrack on
  • SnapToTrack on
  • Spline on

To use:
Draw the line and then backtrack to mask.
