How hard is it to sculpt something like this in Zbrush so that it can be machined on a CNC machine?

Depends on the CNC machine and what type of file it needs for input.
its a 5axis machine
Sculpting such is not super difficult but it’s also clearly no total beginner piece.
The CNC machining of exactly this shape would probably be challenging even with a 5 axis machine.
It would certainly help to split up the finished model into several chunks so that detail rich areas are easier to reach.
Also this parting including creation of pins and slots for precise assembly can be accomplished in Zbrush as shown in
the thread with this picture.
Of course your 5 axis CAM-package had to be able to deal with meshes as input.
