I’ve been struggling with this for a while now. I make an insertbrush from a subtool. Then, when I set drawsize to 1 and insert it on another mesh it has the wrong size. No matter what draw size I set (4 is close but not exact), I can never insert the subtool at it’s original size. It’s important, because it needs to be 3D printed and have accurate size. Off course I press control when inserting the mesh.
I don’t think it works like that. Holding Ctrl will set the inserted mesh to the Draw Size.
For more accurate sizing you may find the IMM Draw Size plugin useful. It’s at the bottom of this page:

Is the picture an accurate representation of what you’re trying to do–duplicating that element at the same size along a planar surface?
In that case it would be easier and more precise to simply create the element as a subtool to the target at exactly the size you need, then duplicate it over an over by control-dragging with Gizmo mode (will duplicate unmasked).
Apologies if this doesn’t apply to your situation.
Thanks Marcus_Civis, IMM Draw size plugin does the trick. It was also suggested by Joseph Drust via Zbrush.
Spyndel, that’s exactly how I go about doing things now. But I wanted to speed up the workflow…
Thanks for replying.
Have a great day!
