check the button size in preferences. reduce it by 5 or so and see if that makes a difference. if it does, resize your document to fit and then save it as a new start up document.
I’m guessing you’re on Windows? You’ll need to right-click on ZBrush and go to Properties >> Compatibility. There, turn on the Override DPI Scaling box, setting it for Application control.
Your operating system is seeing ZBrush 2018 as a different program from ZBrush 4R8 (which it is, despite the name) and so is reverting to its default behavior of upscaling everything – which in turn makes it look like merdhe!
Once ZBrush has control again, the Buttons Size slider will let you fine tune the appearance so that you’re happy again.

I was having the issue with the interface being way too small on the Cintiq Pro 24 with Windows at 100% DPI scaling. I found scaling any higher than 100% would fix the interface being small, but you would have to sacrifice detail/sharpness on the model.
The solution that worked for me was indeed the ‘button size’ setting.
It is located under Pref > Interface > UI. By default, it is set to 47, I believe, which is way too small for a 24" 4k Cintiq. I’ve found a good size to be between 65-80. I settled on 70. =)
