Everything saves and shows up properly from my Custom UI except “Active Points” and “Total Points” from the misc options.
No matter where I drag them too, They disappear on next load.

If you aren’t using ZBrush 4R4 P2 then you should upgrade, see here:

If you are using 4R4 P2 then you will need to create a new custom UI from scratch, not import a configuration from a previous version.
Note that some UI elements only appear when you have a 3D tool in Edit mode.
Hey the update helped solve this problem, thanks for drawing my attention to this. 
Updating didn’t work for me. My entire UI returns to its default entirely with every restart.
I do have two sets of folders:
Program Files (x86)
> ZBrush 4.0
> ZBrush 4R4
Should there only be one folder [latest version]?

I have like 3 sets, so no, I don’t think that’s it. How are you saving the UI? Shift+Ctrl+ i should work. It should state it has been saved.

Thanks, I just spoke to Sablefine (works at Cryptic Studios) and he had the answer:
None of the customizations will hold unless you go to PREFERENCES > CONFIG and select “Store Config”. You can also “Save UI” as a backup.
