Figured out the problem.
Wasn’t aware that the smoothing option for subdividing also turns on and off smoothing for any changes done to a lower subdiv level
It was working fine but zbrush suddenly started working weird for me.
When I manipulate the low res mesh, the changes somehow just remained faceted instead of smoothing along when I go back to higher subdivs.

Did I turn some option on or is zbrush just freaking out?
it doesn’t seem to affect any new tool I create or if I append something new into the current tool.
