Did this begin as a base mesh? Any inner mouth geometry yet? What program is used for the rendering? Is this for a short film, or just exploring workflow / software? Sorry for so many questions. Work of this high quality makes me insanely curious. How does it all click together?

Yeap, a very simple one (about 1000 tris, almost the same as the one I used to sculpt tht Old Hunter, another character I posted here a few months ago), done in 3ds max

I used 3ds max 9 and Mental Ray as the render with SSS Fast Skin shader. This is not for a short film, but for my portfolio and pleasure,
I really but really enjoy sculpting in Zbrush! The final image is going
to be a 3D illustration.

Don´t be sorry, go ahead and ask anything, that´s the idea here And for me, something that really helps me improve my work is praticing A LOT digitally but also drawing and sculpting traditionally.

That is outstanding work! I really love your sculpting and render at the bottom.
I have a few questions if you dont mind. I am still new to zbrush so forgive me if my questions seem basic. I am also a traditional sculptor who has made the switch to zbrush and I love it!
1 - I would like to achieve that same style of render in my work. Did you polypaint your sculpt in zbrush and import into to max and render? I tried a basic render in max 9 last night of a new head sculpt but it was basic and I didnt have any textures so it looks flat. Can you give a brief expanation on how to render a model to achieve the look you did in that last image? It looks so real.
2 - One of the things I have the most difficult with is the areas around the eyes, nose and mouth. FIne wrinkles also give me problems sometimes. Can you offer any tips on getting fine wrinkles and details?
3 - Any tips on how you made those great eyes?

render looks great but my only observation is that some of the wrinkles that you have around the face arent necessarily correct. I would recommend to look at some pics of old people and wrinkle faces while youre working to see where and how the wrinkles work, speciallu around the eyes.
