Two issues:
First is actually an issue that has existed since at least 3.0 and that is buttons disappearing from custom menus. The following buttons have repeatedly randomly disappeared from custom menus, I believe they don’t save properly or load properly but it seems unpredictable when it occurs: Edit:Tool:Undo,Redo; Preferences:Misc:ActivePoints,TotalPoints; everything in the submenu containing Transform:Activate Symmetry.
Second, and more egregiously: using Tool:SubTool:DeleteSubtool from a custom UI consistently crashes ZBrush. I’ve tested this on two machines. Dragging this button anywhere then using it produces a crash.
Specs: System 1: Windows 7 Pro 64-bit, Intel Core i7, 6GB
System 2: Windows Vista 32-bit, Intel Core2Duo, 4GB
