Try this:
Sculpt with a brush of your choice. Then use Shift to smooth out some regions. After Shift-Smooth the Smooth brush is still displayed in the left tray although the previously used brush is in use again.

Please follow this forum’s guidelines when posting any troubleshooting inquiry. With each troubleshooting post, please include your full system specs and any other pertinent information which will allow us to determine if this is one of the following: a) user error? b) Misunderstanding of a specific feature, such as its usage and limitations? c) An item which has already been resolved. d) An item which needs to be resolved.
IMPORTANT: Before posting a troubleshooting item, make sure that:
You are using the latest ZBrush version.
You have a ‘clean’ ZBrush install. i.e. remove any items which you have added (such as older ZScripts and other startup items) which may result in unexpected - and difficult to resolve - problems.
You did not increase the PreferencesMax polygon count or Compact Mem to a level which can not be supported by your system specs.

Uhh, no offense intended Aurick, but it appears to be a bug report, a minor cosmetic one at that. I’ve noticed the same thing, though personally I didn’t find it worth mentioning. What is the preferred forum for bug reports anyhow?
Also, just an idea, but you could put a section in user profiles for filling out system specs, and have it appear under their name on the left, similar to the Guru3D forums.
For ANY bug report to be valid, we need a uniform baseline. I do not know from the OP what system specs are involved, whether any plugins have been installed, what the memory settings are at, or anything like that. This makes it extremely difficult to try and verify a problem, or to resolve it once verified.
If you anticipate making quite a few troubleshooting posts, you can also modify your forum signature to include basics like system specs. I will pass the suggestion on to the webmaster, though.
After restarting the system the effect is gone. I don’t know which steps led to this display error.
I have installed the lastet ZBrush update (3.1). I have two plugIn installed: ImagePlane3 and Quicksave.

I get the same problem frequently as well.
I believe that it is tied to assigning hotkeys to brushes and then using the shift, as others who don’t use hotkeys tell me they never get the problem. I also believe that the reset brush script solves the problem, as does reinitializing Zbrush.
