When I choose the smooth alt brush it doesn’t work anymore in 21.5.1
The cursor circle becomes blue when holding down shift but it doesnt move when I brush and it doesnt smooth anything.
Regular smooth and smooth stronger etc. work without problem.

I can’t immediately reproduce your issue. Please make sure you are using the brush on a mesh with enough features to show it off, like the default dog.Ztl from Lightbox. Attempting to use it on something like a sphere might not show much effect.
