Need Help-Weird triangle appeared on Model

ZBrush Document1.jpg have been working on model only using edit mode,i work my way up sub divisions adding detail and hide parts of body and work on a small section at a time.
i finished at lunchtimne and save file i have been working this afternoon and unhide all before i saved and noticed this big triangle even when i drop to level 1 there is still a problem(looks like a point has been dragged all across screen)
How did this happen?
And how can i correct this please?

take it back to maya//max , fix the vert. kill half the model and weld it to a new half… then pray some. take into z again and … hope for the best.

If you ever get that on one side of the model use smart sym from the good half from lowest sub level upto highest and it will be gone. Other option is to delete the sub level that is normaly highest. Bad for you that its on both sides only on 3rd picture shows on one side in which case smart resym will work.

I do not really understand how to resolve this.
1.If i take it back to max do i just export the lowest sub div, delete half, mirror and take back to zbrush.
2. What the higher divs?
Tez, could you please explain further what you mean.you are right in what your saying that it appears to be coming from one side on lowest div.I am relatively new to zbrush and really do not want to start over on this character-i have spent alot of time working on her
can anyone explain why this happened as i am careful to only ever use edit mode and work on small parts at any one time

Asuming one side is perfect you need to mask sellect the good side right down the middle ctrl + drag over this area. One side should show grey the good side.

From the tools menu open the tab deformations. Be at a low sub division level and press Resym then smart resym. Go up to the next higher sub level and again press resym then smart resym. Keep on doing this untill you reach the highest level or to the point you see the bad deformation vanish. Its worked this way for me many times.

I also normaly resellect the half I want again on each sub level just in case not all the avalible polygons for the next level up got sellect correcly down the centre line of my model.

tez, no joy im afraid,followed your advice but zbrush crashes each time i try to go up or down a level after i have masked selected good side and pressed Resym then smart resym.All goes fine up until i applied deformations but when i try to go up and down sub div, zbrush crashes,i have even tried it with the meask unselected.

Hmm sounds like its very messed up. Normaly zbrush crashes when you try to open such files not when you change sub levels. How low is your lowest level could you do wihtout that level and maybe delete it?

How big is the file I could try to fix it.

whilst waiting on tez, i decided to export sub div level 1 model back to max.
the triangle present turned out to be a vertex from her little finger that somehow had attached itself to the front of her hat.
I fixed this and imported back to zbrush replaced old level1 with fixed one,
however when i went up through the sub div i got weird results as sceenshots show-

I am guessing that the problem here could be with the obj.Exporter settings in max 8
the settings that i checked are-
Group and material-group by Object
Geometry-rotate model
Faces-Quads
Texture cordinates
normals
smooth groups
vertex scale 1
file-# of digits 6

Everything else remained unchecked.

Here is update of work.2nd time using zbrush,quite pleased so far only just started using projection master,working on face first,will be adding more detail all over
One question - can i use material in zbrush on this model?This level 6 will be the normal map for my lopoly which i will create diffuse and spec maps in painter and photoshop(also looking at a good skinshader i just got,getting ahead of myself)
