Looks like you used a brush without back face masking and then re-dynameshed.
Basically your brush is sucking up the geometry from the other side of your mesh making it paper thin.
Under: Brush < Auto Masking < Backfacemask should solve your problem. I have had this problem a few times but don’t always realise until I’ve done a bunch of other work so I don’t want to undo back to before I did this to fix it. What is the best way to patch these holes up?
I tried using the clay build up brush but it never closed it off.
