How could you make custom brushes like that?

Alright folks, I’ve recently bumped into an episode of the “Ask Zbrush” series, which made me ask more questions, though not related to the actual topic. Basically, the link below explains how to create morph target layers but what made me truly fascinated was the spike brush demonstrated by Joseph Drust.

Can anyone share the process of its creation? I tried to make a similar one with the plain 3D > sculpting > alpha grab doc formula but I just want to learn if there is a better workaround.
