How come when I set up my canvas size and background gradient, then save the configurations, the next time I load up Zbrush it doesn’t save these settings?

You have to save the Document. After you make your settings in the Document menu and after you “Resize” and answer yes to the popup dialog, click “Save As Startup Doc” button.
