Question is in the title.
How to get it always activated and from Zbrush Start up.
for some reason it also gets deactivated at some point during a same sculpting session, how come?

Good question. It bugs me too. The only thought I had was saving it out as default for each brush, but that see,s cumbersome.
You can place the Backface Mask button in an accessible location on your UI, or assign it a hotkey to make toggling it on and off relatively painless.
Ah, like the hotkey option.
