Sculpted in ZBrush, the color texture was painted in Substance Painter, rendered in Maya with Arnold, used Xgen core for the hair.

It is not a mistaken observation that his face was swollen, he stated it himself in two letters:-
Tuesday, 21st November, 1848 to Woj ciech Gryzmala
“I have neuralgia and my face is all swollen”
Wednesday, 22nd November, 1848 to Solange Clesinger (nee Dudevant)
“My face is swollen with neuralgia, I can neither breathe nor sleep.”
I would also point out that Chopin’s eyes were brown not grey and his hair was auburn. On all his known images, his mouth is depicted as small and his nose dipped and tapered to a point. His forehead had an obvious indentation across its width, probably from where his hat would sit. The clothes attached to this image are clearly modern. The use of a much refined death mask, & a photograph that is assumed to be Chopin but there is no absolute confirmation or provenance, are strange choices to make when doing this work. So, although it is very clever, it is not a realistic representation of how Chopin really looked.
