Looking at the point count should be the answer to that one. Why do you have freeze subdivisions enabled? Did you sculpt on the mesh and then ctrl drag in the workspace, after changing the value?
I click Dynamesh. It says The mesh has multiple subdivision levels
Would you like to freeze subdivision level before entering Dynamesh mode. I click no. Then it still comes out blocky the same at max res Dynamesh.
It goes like this straight away. Without sculpting then ctrl drag.
How do I view the point cloud please? The Zbrush online documentation appears to be down for me.

Look in the upper left, it is displayed(I don’t have ESP and can only go but what is in your picture and text descriptions.
You should have a well organized workflow, as well as minimal points needed to block in the form. DynaMesh is great for some stuff, if you need to use it, use it. If not, I ask why?

If you have time could you please try applying Dynamesh to it?
The Active points are 13 Mil when I try to apply Dynamesh and it goes down to 1000 active points when applying Dynamesh at full res.

Here is more; 13 Mil is one heck of a lot of points and there should be serious details in the mesh before then.
Not quite 110K points in this wireframe. No DynaMesh was used. Pre-4R7 and new tools to learn. You have a Documentation folder with a bunch of .pdf files
Ideally, your mesh should fill the Tool > Preview window. You can go to Tool > Deformation and click Unify to do this quickly.
Many thanks for all your help. Have been going through as many tutorials as possible. Sometimes I just get stuck. Much appreciated. Fixed it. Sent it Goz into 3ds max and it had a small bunch of polygons way away from it but still the same object. Deleted them. Send it back to Zbrush decimated it. Then Dynamesh was working fine. Thanks for the guide on Active points. Will follow that.
