Here is a breakdown of how one model was constructed:

It starts with the Zsphere model. the resolution display was turned down to allow me to see if some spheres were bunching up on each-other.
An adaptive skin was made at a density of 1. This was then modeled. It's easy to work a model when the resolution is low.
Once that stage is taken as far as it can go, the whole model is Divided (modifiers) and the modeling continues.
The model is divided again and edited. To get more detail in the head but not the body, it was masked, then divided.
The model could have been divided again, but I decided to keep it light in case I wanted to export it for use in animation.
