Teeth Extrude Problem

Hi i am new user to zbrush . and i am in love with this tool .. i will really thank full if some help me out in these problem i am facing ..
1( extruding Teeth like we see in dino tutorial .. i dont know where is that option which let u paint alpha first and then extude ..( if someone can help me by writing simple 2 o 3 lines instructions)

2) if once i am out of edit mode how can i go back ...
