When using zspheres (and I assume any other fuction with x symetry turned on) I seem lose the symetry and the model gets shifted to one side slightly. this cause the symetry to be offset, and not work (the second cursor doesn't land on any zspheres). Am I causing this accidentally or is this a bug?

This is a common problem known to happen when many small changes are being made to your model.
I also find myself accidentily hitting the z and x symetry buttons when trying to undo something (ctrl Z). I now check my transform drop down often to ensure I have the right stuff activated.
You can try the resym and smart resym buttons in the tool window to remedy any mess ups, but this may cause problems if you have already started to sculpt your adaptive skin.
