User Maat Ra, if you are referring to the extra glossiness of the lips it's very easy! (Provided you have the commercial version, that is, and not the demo.)

First, set the base skin's material to exactly what you want. Then save that material. Next, choose a material that you don't plan to use in your image. Then load the material that you just saved. This creates a duplicate of the original material. In this duplicate, increase the specularity and maybe modify the specular curve. Then paint the new material (with just M active in the Draw palette -- no depth or color) onto the lips. Provided the material isn't TOO different from the previous one, the two materials will blend together nicely and you'll have glossy lips.

You can further enhance this effect by setting up your lighting and render settings how you want them, doing a Best Render, and then baking the image. Next, take the 2D enhancer brushes to the lips (such as Shading Enhancer or Highlighter) to really make the glossiness stand out.
