I speak very badly English but I am going to try to help you of best that I can.
- You make a hole for the place where you want to make the spotlight in edition then you mark your object (Press M with your keyboard to mark your object).
- You add a sphere (And mark it) and in the options of material, you add to him of Glow.
- assemble your objects on the scene with the tool " Multimarker ".
- then assemble all this with "Make polymesh".

You can delete polygons, but ZBrush does not have a way to tunnel through a polymesh object.

A non-mesh way to do what you'd like is to snapshot your model to the canvas in the position that you want it and then use a cylinder3D with Zcut active in the Draw palette. This will tunnel through the object, but the object cannot then be rotated (it's a pixol effect, rather than polygon modeling).
